If you are making a kind donation to help us in our work, please leave the "Card Recipient" and "Baby's  /Individual 's Name" boxes blank.

If you wish to make a donation in memory of a precious baby or individual, 1st Breath will send a memorial notice to the individual you designate as the card recipient.  Please type recipient's name and mailing address on the first line using commas for return points. (Name, Street Address, City State ZIP). On the second line please enter the name of the individual who is being remembered and honored.

If you would prefer to use Zelle, please direct to 816-830-9572.  We would appreciate an email or text providing contact information for mailing your receipt and/or memorial donation notice for those being honored.

If your business participates in an annual United Way campaign, please consider designating part of your donation to 1st Breath by attaching the United Way's donation designation form and listing 1st Breath.
